Item 1. Basic personal data
2.2吋證件照電子檔(※請上傳一般證件用照片，勿上傳生活照，檔名為學號，檔案類型為jpg，檔案大小不得超過1MB)
Item 2. 2-inch digital photo which is used in licenses (Attention please, please upload normal license usage digital photo, DO NOT upload photos from daily life. The file name must be your student ID number, the format of digital photo must be jpg, photo size SHALL NOT bigger than 1MB.)
3.郵局帳戶
Item3. Account of deposit book of Chunghwa Post Co., Ltd.

登入方法：新生基本資料登入系統的登入學號、密碼為校園入口網之帳號密碼(若未曾登入過校園入口網的人，密碼為身分證(英文大寫))
Login Method: The log in student ID number and Password for database of freshman’s basic information are same with the username and password of campus portal (If you never log in campus portal before, the password will be your ID card number (English SHALL be capital letter)).

因應個人資料保護法措施對未來新生入學註冊組取得新生基本資料之做法說明新生入學需上網登錄個人基本資料，在登錄前請讀完以下之聲明後，開始填寫個人基本資料
 Measures in response to the Personal Data Protection Act, the registration division would like to explain how to get the basic information of freshman who got admission to MCUT in future. You must register your personal basic data online when you get admission if you are a freshman. Before registration, please read carefully following statement, tick agree if you done, after that please log in and fill up your personal basic data.

本校依「個人資料保護法」規定，錄取本校之學生所提供之個人資料，供本校入學學籍等校內行政相關事務人處理及利用，非取得本人同意或其他法令之特別規定，不會任意將個人資料揭露於第三人或使用於蒐集目的以外之其他用途。
MCUT complies with the provisions of the "Personal Data Protection Act", students provided personal data who got admission to MCUT only for related divisions and workers that use and handling for internal administration. MCUT won't disclosure student's data to third party or another usage what is not for collection unless student own self agrees or another special regulation in Laws.

     After reading, I fully understand contents that it is comply with requirement of "personal Data Protection Act", and agree MCUT collect, handling and use my personal data, this agreement has legal effect.
